Smart eR 1095 Notice:

Tax Form 1095-C:  Employees can access form 1095-C for tax year 2025 through their Smart eR accounts.  The 1095 form can be found in Smart eR under "My Payroll" and "ACA Form 1095".  Choose the electronic version to view and print the form.  If you would like to request a paper copy or do not have access to Smart eR, please contact the school at 218-283-2571 ext 1182.
